How To Become Our Client

There is no contract or written agreement you must abide by… no rules, no hidden cost. You do not have to be classified as “homebound” to participate in many programs.

  1. You must first qualify for in home-services through one of the Medicare or Medicaid providers, who assist us with meeting each client’s ongoing needs.

  2. You must answer a few questions. We only schedule home visits after a brief phone interview is completed. Once we determine your needs and the programs you qualify for, our advocates will know which forms or applications to complete on your behalf. They will then schedule a time to meet with you/ and or your family.

Medical Programs

We assist with Medicare, Medicaid, HMO’s, Drug Plans, Scheduling Sleep Studies, Spendowns, Insurance Premiums, Durable Medical Equipment, including diabetic shoes, incontinent supplies (diapers, pads, etc.), and wound care supplies.

Health and Food Services

We assist with obtaining Food Stamps, (Chore Workers) Consumer Directed Services, Home and Community Services, Adult Day Care, Meals-on-Wheels, Home Delivery of medications, transportation, affordable housing, utility assurance programs, government phones, access to food pantries, non-covered medical supplies and Skilled Home Health Services for Medicare and Medicaid recipients.

Included Disciplines

We provide Skilled Nursing as well as Physical, Occupational, Restorative, and Speech Therapy. We also provide referrals to home visiting physicians such as, Primary Care, Pain Management, Podiatry (feet), Urology (bladder) and Wound Care Specialist’s.
